Transition Youth Planning Services are youth-centered planning meetings that CFI facilitates for youth in out-of-home placement such as the State Training School or residential facilities, prior to their return to their home community. Youth are referred from Juvenile Court Services and facilitation focuses on providing support and resources needed to ensure successful re-entry and future planning.
