Integrated Health Home Services

Children & Families of Iowa’s Integrated Health Home program is designed to help families of children who have a mental health diagnosis navigate their healthcare and improve their overall health and wellness. With access to care 24 hours a day, seven days a week, the program provides a single point of contact for all aspects of a child’s health care needs, including physical, mental and social support.

Managing quality mental health services a child needs can be a stressful process. Children and Families of Iowa’s Integrated Health Home program is comprised of health care professionals working together to help develop a plan dedicated to meeting clients’ health care needs. Once the plan is put in place, Children & Families of Iowa will help navigate health care services and provide connections to community resources.

Who is eligible?

To be eligible, participants must have a child age 0-18, with a mental health diagnosis such as ADHD, depression or anxiety, and participate in the Medicaid program. Participants must also live in one of the following counties:

  • Calhoun
  • Hamilton
  • Humboldt
  • Kossuth
  • Pocahontas
  • Webster
  • Wright

How the Integrated Health Home program works.

  1. Families are partnered with a Care Coordination Team, composed of a Nurse Care Coordinator, Care Coordinator and Family Support Specialist, who will work with parents to ensure their child is receiving the best care possible.
  2. The team collaborates with health care service providers to meet the child’s needs. Providers may include physicians, therapists, schools and social service agencies.
  3. The team partners with families to develop an individualized plan that meets the client’s health care needs. This integrated approach is unique in that it works to improve a child’s overall health by considering all aspects of care.
  4. The Care Coordination Team will continue to provide additional support, helping families connect with resources and manage services included in the plan.
